If the person authorized by the claimant submits the request form as the claimant's representative, please submit a letter of attorney or enclose a copy of the claimant's identification documents.
If a legal representative is submitting the request form as the claimant's agent, please enclose a document (such as a copy of the family register) that proves the qualification of the legal representative.
In this case, a copy of the claimant's identification document is not required.
About disclosure fees
There is no disclosure processing fee.
Result of application for disclosure, etc.
The results of the disclosure of personal information applied for by the person in question will be reviewed, and after settlement, a "Personal Information Disclosure Notice" will be mailed by registered mail as a response.
